FAQs

  • Horizons serves the families of children, adolescents, and teens from ages 2-18. While our strategies can be applied to both neurotypical and neurodiverse individuals, our team of Behavior Analysts has years of experience working with children and caregivers who need individualized care, including those with diagnoses such as Autism Spectrum Disorder, Attention Deficit Hyperactivity Disorder (ADHD), emotional and behavioral disorders, and Oppositional Defiant Disorder.

  • Our services are meant to be flexible, as we recognize each families’ needs are different or change over time.

    Primary areas of support include:

    • Increasing independence in daily routines

      • handwashing, toilet training, bedtime routines

    • Increasing success in community outings

      • doctor visits, haircuts, trips to grocery store

    • Facilitating communication methods

    • Understanding or helping others to understand the individuals’ diagnosis

    • Managing and decreasing maladaptive behaviors

      • hitting, biting, self injurious behaviors such as head banging or skin picking

  • Horizons offers services through Wisconsin’s Children’s Long-Term Support (CLTS) Waiver Program which provides services to children with developmental, cognitive, and physical disabilities.     Horizons also accepts Medicaid, most primary insurance providers, and private pay.

    Rates for Caregiver Support Services range from 45-180 dollars depending on duration of services and providers involved.

  • For insurance funded services, select the “Get Started” tab below and fill in your contact information. A team member from Horizons will then reach out to move forward.

    For CLTS services: After ensuring your child or teen is approved for CLTS support, services can be sought out by caregivers or referred through county caseworkers. Caregivers can use the “Get Started” tab below to submit their information. Caseworkers can fill out a referral form and submit it to info@horizonsbehavioralhealth.net.

Interest in Groups?

We are considering and hoping to launch groups such as social skills, daily living skills or recreational/leisure groups designed to help individuals with autism and developmental disabilities enhance their social interactions, to foster relationships within the communities that the individual lives in and to HAVE FUN! We would love to hear from you!
